29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a) At the establishment, employees performing janitorial services used concentrated and diluted Virex II 256 from spray bottles. Virex II 256 is a corrosive and can cause permanent eye and skin damage including blindness. Suitable facilities for the quick drenching or flushing of the eyes and body were not available.

29 CFR 1910.1200(f)(6): Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked: a) At the establishment, employees performing janitorial services used concentrated Virex II 256 from a spray bottle labeled as containing Glance HC. Virex II 256 is a corrosive and can cause permanent eye and skin damage including blindness. b) At the establishment, employees performing janitorial services used Virex II 256 from a spray bottle labeled as containing Glance HC. Virex II 256 is a corrosive and can cause permanent eye and skin damage including blindness. c) At the establishment, employees performing janitorial services used Stride Floral SC Neutral Cleaner from spray bottles labeled as containing Glance HC Glass & Multi-Purpose Cleaner. Stride Floral SC Neutral Cleaner is irritating to the eyes and skin. d) At the establishment, employees performing janitorial services used Virex II 256 from a spray bottle labeled as containing Crew NA. Virex II 256 is a corrosive and can cause permanent eye and skin damage including blindness. e) At the establishment, employees performing janitorial services used Glance HC Glass & Multi-Purpose Cleaner from a spray bottle labeled as Alpha HP. Glance HC Glass & Multi-Purpose Cleaner can cause eye and skin irritation.

29 CFR 1910.1200(h)(3): Training. Employee training shall include at least: 29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information. a) At the workplace employees used cleaning chemicals such as, but not limited to Virex II 256 and Stride Floral SC Neutral Cleaner from improperly labeled spray bottles. Employees had not been trained on the requirement to properly label chemical containers exposing employees to contact and inhalation hazards.
