29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 3/15/18, the employer failed to ensure that employees (slitter helpers) assigned with placing cardboard strips onto the take up reel of the Line #2, Paxson Slitter, removed the key from the control panel after disabling the Operator's panel.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 3/15/18, the employer failed to ensure that employees (slitter helpers) assigned with placing cardboard strips onto the take up reel of the Line #2, Paxson Slitter, removed the key from the sub-panel and followed the following steps during setup: 1) Prepare for shutdown, 2) Shutdown machine or equipment, 3) Machine/equipment isolation, 4) Apply lockout device(s), 5) Relieve potentially hazardous energy sources, and 6) Verify energy isolation.
