29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. At the compressed gas cylinder storage rack outside the South overhead door of the main building, oxygen compressed gas cylinders were stored within 7 ft. of acetylene compressed gas cylinders.

In the Warehouse Building on a table where a coffee pot and microwave are located both devices were plugged into a six receptacle surge suppressor which was then plugged into a 110 volt AC two receptacle outlet and not in accordance with the listing and labeling for the surge suppressor.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. In the Warehouse building above the table where a coffee pot and microwave are located, employees were exposed to an electrical shock hazard from a 110 volt two receptacle outlet, which was missing its faceplate.
