29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):   a)Throughout facility: The employer did not maintain a written certification of each powered industrial truck operator's evaluation that included the name of the operator, the person conducting the evaluation and the date of the evaluation, on or about 3/22/18.      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.    The certification of forklift evaluations shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.

29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed:   a) P&H 5 ton crane above plate roller: Monthly inspections of the hook on the crane were not documented in a certification record that included the date of the inspection, the signature of the person performing the inspection and the identity of the hook. The safety latch was missing from the hook, on or about 3/22/18.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:  a) Throughout facility: Periodic inspections of  two P&H 5 ton cranes and the two P&H 7 ton cranes were not conducted at least once every 12 months, on or about 3/22/18.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.    PERIODIC INSPECTIONS OF THE CRANES MUST TAKE PLACE IN ONE TO 12 MONTH INTERVALS DEPENDING ON CRANE USAGE, SEVERITY OF USE AND ENVIRONMENT. THE INSPECTIONS MUST INCLUDE THOSE ITEMS LISTED IN 29 CFR 1910.179(J)(2) AND (J)(3).
