On or about March 21, 2018 in a hotel at 150 S Nicolet Road in Appleton, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the electrical hazards associated with employees using lighting suspended from fixture wires without bulb protection, light switches and outlet receptacles without plate covers installed, and working around open light socket.
