29 CFR 1910.30(a)(1): The employer did not provide training for each employee exposed to fall hazards and who used personal fall protection systems or is required to be trained as specified elsewhere in this subpart prior to May 17, 2017:  Camp Del Mar CLB-5 Compound: Effective training on fall hazards and the proper use of personal fall arrest systems was not provided to employees working at heights. Employees work in aerial lift trucks performing maintenance tasks, such as repairing lighting systems. Employees were exposed to fall hazards.

29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  Camp Del Mar CLB-5 Compound: On March 19, 2018, an employee was performing maintenance on the overhead lighting from the bucket of an Altec aerial truck without the leg straps of his personal fall arrest system secured. The employee was exposed to fall hazards.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Facilities Maintenance Division / Camp Del Mar: An assessment was not conducted to determine the hazards associated with electrical-related work and the associated personal protective equipment that would be required for the task. Employees were exposed to electrical shock hazards.  MCI West - MCB Camp Pendleton Facilities Maintenance Division was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.132(d)(1)], which was contained in OSHA inspection number 1288750, citation number 1, item number 1 and was affirmed as a final order on April 25, 2018, with respect to a workplace located at Building 2296, MCB Camp Pendleton, California 92055.
