29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles.    a. On or about March 22, 2018, at the above referenced worksite, employees were performing new construction on a residential structure. Employees operating pneumatic nail guns and / or in close proximity to these operations, were exposed to struck by hazards and were not protected by safety glasses. Employees were thereby exposed to eye injuries.    There is no abatement certification of documentation required for this item.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.   Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).     a. On or about March 22, 2018, at the above referenced worksite, employees were performing new construction on residential structure. An employee working in a GENIE S-65 telescopic boom aerial lift was not properly attached to the boom or basket by the use of a lanyard, thereby exposing the employee to the hazards associated with a fall of more than fifteen (15) feet.     There is no abatement certification of documentation required for this item.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.    a. On or about March 22, 2018, at the above referenced worksite, an employee was exposed to fall hazards while improperly using an A-frame ladder to  perform new construction on a residential structure.  The employee was using the A-frame ladder in a folded up position as it was leaning against the wall.      There is no abatement certification of documentation required for this item.
