29 CFR 1910.333(a): "General." Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards. On or about February 9, 2018, the employer failed to ensure electrical safety-related work practices were employed to prevent an employee from receiving electrical shock injuries while disconnecting the electrical conductors of a Bridgeport milling machine from an energized 120V or 277V electrical circuit.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about February 11, 2018, the employer did not report the in-patient hospitalization of an employee who sustained an electrical shock injury while working on an energized circuit within 24 hours. The employee was admitted to the hospital and burn unit for treatment of third degree burns and had been away from work for at least 30 days. OSHA was not notified until March 22, 2018 when the current inspection was opened.
