29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about March 21, 2018, and at times prior thereto, at the establishment the employer did not establish a Lockout/Tag-out program consisting of an energy control procedure, employee training and periodic inspections in maintenance testing operations.  Employees were exposed to unexpected release of energy hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about March 21, 2018, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:     1) At the hydraulic press brake area, a press brake being operated via foot pedal without machine guard provided to protect operator and other employees from exposing to point-of-operation while in operation.     2) At the manual lathe area, a manual lathe being operated without chuck guard provided to protect employees from contacting the chuck while in operation.    Employees were exposed to struck by and caught in hazards.

29 CFR 1904.41(a)(2): 29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     On and or about March 11, 2018, and at times prior thereto, at the establishment the employer did not complete a written hazard communication program to describe how the company would meet the requirements of employee training, gathering SDS sheets and labeling containers.  Employees were exposed to possible physical and/or health hazards due to exposed to various hazardous materials.
