29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about March 15, 2018 and at times prior thereto, lockout or tagout devices were not attached to the #9 Silo unloading conveyor prior to employees entering the #9 silo to sweep it out. This condition exposed employees to struck by hazards from the auger inside of the silo.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: On or about March 15, 2018 and at times prior thereto, the lockout procedures for the #9 silo did not include procedures for group lockout. Employees who entered the #9 lockout were not afforded the same level of protection as if they had used their own locks.
