29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights. On March 20, 2018, at this residential construction site, employees engaged in painting preparation work on the exterior wall of a multifamily-residential building. Employees utilized a scaffold that was not fully planked or decked, employees were exposed to fall hazards.

29 CFR 1926.451(f)(15): Ladders shall not be used on scaffolds to increase the working level height of employees. On March 20, 2018, at this residential construction site, employees engaged in painting preparation work on the exterior wall of a multifamily residential building. Employees utilized a four foot tall step ladder on a scaffold to increase the working level height, employees were exposed to fall hazards.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On March 20, 2018, at this residential construction site, employees engaged in painting preparation work for the exterior of a building. Employees utilized a scaffold that did not have a form of fall protection. This exposed employees to fall hazards greater than 10 feet.
