29 CFR 1910.132(f)(1)(ii): The employer did not provide training to each employee who is required by this section to use PPE on knowing what PPE is necessary:  Floor trusses building- On or about March 22, 2018, and at times prior to, employee failed to provide training to employees removing truss connector plate from a truss with the claw of the hammer while hitting it with a second hammers face with no adequate personal protective equipment or training.

On or about March 22, 2018, the employee did not record the following workplace injuries and illnesses correctly on the OSHA 300 Log for calendar year 2017.   a) On or about March 16, 2018, Laborer - an employee was struck by a piece of metal while removing a metal truss connector plate, resulting in permanent damage to the left eye, resulting in days away from work. The employer did not enter an estimate for the days away from work.

The employer did not classify the incident.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.  a) On or about March 22, 2018, employer did not verify that the 300 logs were free from errors before certifying the Annual Summary and posting it in that there were two injuries in Calendar Year 2017, one of which was not classified and therefore not included in the section entitled "Number of Cases".
