29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about March 22, 2018, employees were exposed to a fall of approximately 17 feet. Employees were performing roofing activities on an existing commercial building located at 3004 Payne, Cleveland, Ohio 44114. Employees were working near the edge of the roof without having a safety monitor in place.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: On or about March 22, 2018, employees were exposed to a fall of approximately 17 feet. Employees were performing roofing activities on an existing commercial building located at 3004 Payne, Cleveland, Ohio 44114. The warning line system was not set back at least 6 feet on the entire perimeter of the building.
