29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures.  This was most recently documented at 17971 Oak Plaza, Omaha, Nebraska on March 23, 2018 when employees were observed installing weatherproofing materials to a residential structure while working greater than 6 feet above the ground without fall protection.  The roof height was 9'6" and the pitch was 5:12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is exposing employees to a fall hazard while performing roofing operations.  This was most recently documented at 17971 Oak Plaza, Omaha, Nebraska on or about March 23, 2018 when employees were using a ladder to access a residential roof, and the ladder side rails did not extend at least 3' past the upper landing surface. The ladder only extended 2' past the roof eve.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  The employer is failing to protect employees from fall hazards when performing roofing operations on residential structures.  This was most recently observed at 17971 Oak Plaza, Omaha, Nebraska on March 23, 2018 when two employees were observed using an extension ladder to access a roof and the ladder had been taken apart.  The employees were using one half of the extension ladder to access the roof which was 9'6" high.  Since the ladder was not being used as it was intended, its strength and structural integrity was unknown, therefore exposing employees to a fall hazard.
