29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The employer is failing to protect employees from struck by and crushing hazards associated with servicing and maintaining equipment with stored energy. This was most recently documented on March 22, 2018 at the facility located at 431 A Street, Lincoln, NE 68501 where the employer has developed inadequate written specific procedures for the maintenance of the bridge tags on the fleet of Mack concrete mixer trucks. The procedures do not identify gravity and hydraulic pressure as potential sources of stored energy nor do they provide instruction on methods to secure the bridge tag to prevent it from swinging down and striking an employee.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): The employer is failing to protect employees from struck by and crushing hazards associated with servicing and maintaining equipment with stored energy. The most recent occurrence of this was documented to occur on March 22, 2018 at the facility located at 431 A St. Lincoln, NE 68501when an employee was servicing the bridge tag on a Mack 713 concrete mixer truck without using the a means to secure it and isolate it from the forces of the gravity that caused the bridge tag swing and injure an employee.

This was most recently documented to occur on or about March 19, 2018 and dates prior in the workplace located at 431 A Street, Lincoln, NE 68501. A machine specific lockout/tagout procedure was not developed for the this equipment that explains hazardous energy sources, the proper sequence for de-energization, and specific energy isolation points for multiple energy sources such as the multiple electrical motors and conveyor feeds.
