Linden NJ 07036 -Middle of the warehouse. The employer did not ensure procedures developed for hazardous energy control were utilized by employees. Employees engaged in the maintenance and servicing of equipment which included, but was not limited to the Automation Conveyor Belt, did not utilize LOTO procedures on the equipment when performing repairs. Violation occurred on or about March 23, 2018
