29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Facility Wide- On or about March 23, 2018, the employer failed to establish a Lockout Tagout Program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on machines, such as the Resaws, Chop Saws, Log Cutter, and Dismantlers, where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service. When defects were found that affected the safety of the vehicle trucks were not removed from service: a) Facility Wide- On March 23, 2018, the employer failed to ensure that all powered industrial vehicles, such as the Toyota Forklifts, were examined before being placed into service. b) Facility Wide- On March 23, 2018, the employer failed to remove powered industrial vehicles from service that showed conditions that adversely affected the safety of the vehicles. Examinations showed that the Toyota Forklift #92764 that had issues with the steering axle and center bushings, and Toyota Forklift #65039 that had braking problems. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
