29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about March 15, 2018 and at times prior thereto, employees were exposed to fall hazards of 9 feet at the edge of a 4/12 pitch roof, where no fall protection was provided, or used.  NOTE-Abatement certification is required for this item.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) On or about March 15, 2018 and at times prior thereto, employees descended a nine foot roof to an eight foot step ladder below, and used the top and top step of the 8 foot step ladder.
