29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6 : Workplace, 52 Green Pond Road, Rockaway, NJ a) Seven (7) compressed gas cylinders of Nitrogen located under a stairway and adjacent to a walkway in the Engineering Department shop area were not secured to prevent them from falling. Violation occurred on or about March 23, 2018

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) Production Area, workplace, 52 Green Pond Road, Rockaway, NJ 1 - A temporary staffing employee working on the plastic injection molding machine #3 and #4 was exposed to continuous noise levels at 71 % of the Permissible Exposure Limit (8 hour Time Weighted Average sound level of 90 dBA) which is 0.710 times the allowable exposure limit. The action level for employees who work 11.5 hours, is 84.38 dBA. The equivalent sound level of the measured dose is 87.5 dBA. Sampling was performed for 371 minutes on 3/23/18, with the unsampled portion of time considered to represent zero for the exposure calculation. 2 - A temporary staffing employee working on the line at the M-7 plastic injection molding machine was exposed to continuous noise levels at 73.0 % of the Permissible Exposure Limit (8 hour Time Weighted Average sound level of 90 dBA) which is 0.73 times the allowable exposure limit. The action level for employees who work 11.5 hours, is 84.38 dBA. The equivalent sound level of the measured dose is 87.7 dBA. Sampling was performed for 485 minutes on 3/23/18. 3 - A temporary staffing employee working on the line at the Sumitomo Demag N-3 plastic injection molding machine was exposed to continuous noise levels at 66.4 % of the Permissible Exposure Limit (8 hour Time Weighted Average sound level of 90 dBA) which is 0.664 times the allowable exposure limit. The action level for employees who work 11.5 hours, is is 84.38 dBA. The equivalent sound level of the measured dose is 87.0 dBA. Sampling was performed for 477 minutes on 3/23/18, with the unsampled portion of time considered to represent zero for the exposure calculation. 4 - A permanent employee working on the line at the M-18 plastic injection molding machine was exposed to continuous noise levels at 77.0 % of the Permissible Exposure Limit (8 hour Time Weighted Average sound level of 90 dBA) which is 0.664 times the allowable exposure limit. The action level for employees who work 11.5 hours, is is 84.38 dBA. The equivalent sound level of the measured dose is 88.1 dBA. Sampling was performed for 484 minutes on 3/23/18. Violation occurred on or about March 23, 2018
