29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph: a) On or about March 26, 2018 the employer failed to ensure that electrical equipment operating at more than 50 volts were guarded against accidental contact. Enclosures and cabinets were not capable or protecting employees in that the doors were broken off or left open without the ability to close. These control panels were present on the building 118 panel being fed from x-11 transformer. b) On or about March 26, 2018 the employer failed to ensure that electrical equipment operating at more than 50 volts were guarded against accidental contact. The control panel on the building 118 panel being fed from x-11 transformer contained a missing cover over #42 circuit spot.
