29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) On or about March 27, 2018, employees were exposed to cave-in hazards where the east and west walls of the trench they were working in at a depth of approximately 8 feet were not shored, sloped, or otherwise protected from cave-ins. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted: a) On or about March 27, 2018, employees were exposed to the hazard of cave-ins while working in a trench having a depth of approximately 8 feet where a trench box was elevated approximately 4 feet above the bottom of the trench. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
