29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: On or about 29 March 2018, the employer did not ensure adequate documented energy control procedures were developed and utilized for employees assigned to perform servicing, maintenance, or setup on all company machines to include the Chiyoda Kogyo Co., Auto Pipe Bender #512 & #514 . Employees were exposed to unexpected start-up when they did not use Lockout/Tagout on all sources of potential energy. Employees were exposed to caught-in/amputation, struck-by hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 29 March 2018, on all company bending machines to include the Chiyoda-Kogyo Auto Bender machines #512 & #514: employees that were assigned to hand-feed parts into the unguarded point-of-operation and associated danger zone, had their hand(s) and finger(s) exposed to hazards capable of causing amputation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
