29 CFR 1926.501(b)(13):  Each employer engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, or personal fall arrest systems:     The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of re-roofing operations.  Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  The workers were exposed to falls of over 9 feet to the ground below.  This was most recently documented at 12632 Timberridge Dr. Papillion, Nebraska on or about March 27, 2018 when the employer had employees working on the roof of this residence without any form of protective system.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof. Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  The workers were exposed to falls of over 9 feet to dirt and concrete below.  This was most recently documented at 12632 Timberridge Dr. Papillion, Nebraska on or about March 27, 2018 when the employer had employees working on the roof of this residence without any form of protective system.

29 CFR 1926.1053(b)(1):  Were portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer is failing to ensure employees are protected from fall hazards while utilizing portable ladders as part of re-roofing operations.  Workers were observed climbing up a portable extension ladder which did not meet the elements of this standard.  The ladder did not extend three feet above the upper surface being accessed by the employees on this site.  The workers were exposed to falls of over 9 feet to dirt and concrete below.  This was most recently documented at 12632 Timberridge Dr. Papillion, Nebraska on or about March 27, 2018 when the employer had employees working on the roof.

The workers were exposed to falls of over 9 feet to dirt and concrete below.  This was most recently documented at 12632 Timberridge Dr. Papillion, Nebraska on or about March 27, 2018  when the employer had employees working on the roof of a residence accessing and egressing a ladder that was not properly erected.
