29 CFR 1904.40(a): The employer did not provide an authorized government representative copies of requested records kept under Part 1904 within four (4) business hours: (a) U.S. Department of Housing and Urban Development, Minneapolis Field Office - 920 Second Avenue South, Suite 1300, Minneapolis, MN. On or about March 27, 2018, the employer failed to provide copies of OSHA Forms 300 and 300-A or their equivalents, which were requested for calendar years 2013, 2014, 2015, 2016, 2017, and 2018 to date.
