29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a)Impact Construction at 1031 Tanner Court, Tea, SD 57064: On or about and at times prior to March 27, 2018, the exposing employer did not ensure employees were protected from injury in that, two employees were installing floor sheathing on the second floor of a multi-family residence and were not protected from fall hazards. This condition exposed employees to a fall hazard of approximately 11 feet 2 inches to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a)Impact Construction at 1031 Tanner Court, Tea, SD 57064: On or about and at times prior to March 27, 2018, the exposing employer did not ensure employees were provided fall protection training. Two employees were installing floor sheathing on the second floor of a multi-family residence and were not trained in the recognition of the hazards related to falls and procedures to be followed in order to minimize the hazards. This condition exposed employees to a fall hazard of approximately 11 feet 2 inches to the ground below.
