OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards when utilizing damaged safety latches on crane hooks:  a) Production Area - On or about March 26, 2018, the hooks for the Shaw Box Dresser 3 Ton Crane 1 had a damaged safety latch in that it would not effectively close.    b) Shipping Dock - On March 26, 2018, the hooks for the Shaw Box Dresser 5 Ton Crane had damaged safety latch in that it would not effectively close.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the guidelines in the American Society of Mechanical Engineers (ASME) B30.10-2014, Section 10.1.10 Inspection, Removal, and Repair.  The employer must ensure that inspections are being conducted on hooks and hooks found with damage shall be removed.  Repairs shall be approved by the manufacturer or a qualified person.
