29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) On or about 03/26/18, at a roofing jobsite on a house at 25 N. Pearl St., Buffalo, NY:  Employees  were working on the ground doing clean-up work in the vicinity of employees working  above on the roof and on a Ladder Jack Scaffold . The employees were  exposed to the potential hazard of being struck by materials  that could fall off the roof and scaffold.  The employees were not wearing a hardhat.        NO ABATEMENT CERTIFICATION REQUIRED

Pearl St., Buffalo, NY: Employees were working from the approx. 20 ft. long pik of a ladder jack scaffold erected on the northwest side of the of the house. The pik extended over the west support bracket by approx. 6 ft.     b) On or about 03/26/18, at a roofing jobsite on a house at 25 N. Pearl St., Buffalo, NY: Employees were working from the approx. 16 ft. long pik of a ladder jack scaffold erected on the southwest side of the of the house. The pik extended over the west support bracket by approx. 4 ft.    NO ABATEMENT CERTIFICATION REQUIRED

Pearl St., Buffalo, NY: Employees working from  the pik of a ladder jack scaffold erected on the northwest side of the house were exposed to a potential fall of approx.  22 ft.  because no fall protection system was being utilized.     b) On or about 03/26/18, at a roofing jobsite on a house at 25 N. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 03/26/18, at a roofing jobsite on a house at 25 N. Pearl St., Buffalo, NY: Employees used an extension ladder to access the pik on the ladder jack scaffold on the northwest side of the house. The siderails of that extension ladder only extended 31 inches above the pik.        NO ABATEMENT CERTIFICATION REQUIRED
