29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   a. On or about March 27, 2018, on the roof frame, employees operating a Hitachi pneumatic nail guns to perform re-roofing work were not wearing eye protection being exposed to flying debris or nails.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about March 27, 2018, at north side of the residence, employees were exposed to a fall hazard when ascending and descending from a rooftop on a portable ladder which was not extended at least 3 feet above the upper landing surface. The ladder was not secure at its top and a grasping device was not provided.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about March 27, 2018, on the roof frame, employee performing sheathing work from a 6:12 pitch roof was not being protected from 9 feet 1 inches fall hazards by a fall protection system.    b. On or about March 27, 2018, on the rooftop, employees performing framing work from a 6:12 pitch roof are not being protected from 9 feet 1 inches fall hazards by a fall protection system.
