(a) On or about March 20, 2018, at 180-66 Liberty Avenue, Jamaica, NY 11433  The employer agreed on March 15, 2018,  to provide copies of the facility's OSHA 300/300A forms for the years 2015 through 2017 by close of business on  March 20, 2017, but by April 4, 2018, those records still had not been provided.  ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  (a) On or about March 15, 2018, at 180-66 Liberty Avenue, Jamaica, NY 11433  The employer had not completed a written hazard assessment to verify the personal protective equipment required for employees who perform tasks such as spray painting vehicles and vehicle parts in the spray booth using hazardous chemicals, including but not limited to, General Gen Max H2232 2K  Hardener (contains isocyanates, n-butyl acetate, naptha, 2-methoxy-1-methylethyl acetate, xylene) and PPG DCX61 High Solids Hardener (contains isocyanates).  ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life:  (a) On or about March 15, 2018, at 180-66 Liberty Avenue, Jamaica, NY 11433  The employer had not set up a change schedule for employees who perform tasks such as spray painting vehicles and vehicle parts in the spray booth using hazardous chemicals, including but not limited to, General Gen Max H2232 2K  Hardener (contains isocyanates, n-butyl acetate, naptha, 2-methoxy-1-methylethyl acetate, xylene) and PPG DCX61 High Solids Hardener (contains isocyanates) while wearing air purifying respirators with organic vapor cartridges.   ABATEMENT CERTIFICATION REQUIRED PURSUANT TO 29 CFR 1903.19.
