Employees of Apex Rockfall Mitigation, LLC. were exposed to fall hazards in excess of 50 feet while being hoisted on drill basket DR-52 to conduct rock fall mitigation activities in that drill basket DR-52 did not meet the following requirements:      1-  Drill basket DR-52 was missing sections of mid-rails as required by paragraph (e)(6).       2-  Drill basket DR-52 did not have a grab rail inside the platform perimeter as required by paragraph (e)(7).      3-  Drill basket DR-52 did not have smooth edges to prevent injuries as required by (e)(11).      4-  Drill basket DR-52 did not have the weight of the platform and its rated capacity conspicuously posted on the platform as required by (e)(12).      5- The control platform on drill basket DR-52 was not equipped with a guardrail system as required by paragraph (e)(6).      6- The control platform on drill basket DR-52 was not enclosed from mid-rail to toeboard with a solid material or expanded metal having openings no greater than 1/2 inch as required by paragraph (e)(6).      7- The control platform on drill basket DR-52 did not have a grab rail inside the platform perimeter as required by paragraph (e)(7).
