On or about March 26, 2018, the employer did not document that the equipment associated with the anhydrous ammonia refrigeration system complied with recognized and generally accepted good engineering practices, such as but not limited to, ANSI/IIAR 2 - 2008 "Equipment, Design, and Installation of Closed-Circuit Ammonia Mechanical Refrigerating Systems". a) The emergency eyewash/safety shower unit installation outside of the engine room was not readily accessible. b) There was no signage for the meaning of each alarm near the visual and audible alarms outside the engine room. d) The pressure relief devices from the evaporative condensers vented downwards towards the employee walking/working surfaces. e) The pressure relief devices from the evaporative condensers were less than 15 feet above the adjacent grade.
