29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) On or about March 14, 2018, employees were exposed to flying objects while not using appropriate personal protective equipment necessary to protect them from the hazard.  Employees were observed using pneumatic nail guns without appropriate impact-resistant safety glasses while securing building wrap. 29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:  a) On or about March 14, 2018, and at times prior, employees were exposed to fall hazards due to working on a pump-jack scaffold that did not have the pump jack brackets fixed at a plumb and level setting.  Employees were using a pump jack to work on surfaces at angles up to 35 degrees at the third story of a new multi-family residence. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a)  On or about March 14, 2018, employees were exposed to fall hazards due to lack of a guardrail system, safety net system, or personal fall arrest system while engaged in residential construction activities more than 10 feet above a lower level.  Employees were observed using a plywood sheet balanced on the forks of a forklift to transport employees from the ground level to the third story.     (b) On or about March 14, 2018, employees were exposed to fall hazards due to lack of a guardrail system on the ends of a pump-jack scaffold used for residential construction activities more than 10 feet above a lower level. An employee was observed working on a pump-jack scaffold without end guards while installing housewrap on the third story. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about March 14, 2018, and at times prior, employees were exposed to fall from height hazards due to portable ladder side rails not extending at least 3 feet above the upper landing surface.  Employees used a portable ladder on the second story landing to access the third story.  At the time of inspection the ladder side rails extended approximately less than 6" above the upper landing surface.
