29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  Employees engaged in, but not limited to, maintenance and cleaning activities were exposed to fall hazards at the following locations in that the following conditions were allowed to exist.  A.  The ladder in the dry mix area that serves as a maintenance platform for the large bulk bagger had a platform that was approx. 8'8" tall and was not provided with a swing gate at the top of the ladder opening.   B.  The ladder in the dry mix area that serves as a maintenance platform for the 101 bagger had a platform that was approx. 6'6" tall and was not provided with a swing gate at the top of the ladder opening.
