29 CFR 1917.27(b)(1): Immediate supervisors of cargo handling operations of more than five persons did not satisfactorily complete an accident prevention course:    a) Sparrows Point Ship Yard Pier - Employees were involved with the unloading of steel railroad rails and plywood from the M/V Pacific Light, where hazardous conditions existed, including but not limited to; laborers being under suspended loads and where landed loads were not contained to prevent movement and the supervisors had not completed an accident prevention course, on or about 03/18/2018.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1917.44(m): Cargo, consisting of pipe or other products which could spread or roll to endanger employees, was not contained to prevent movement when flatbed trucks, platform containers or similar conveyances were loaded or discharged:    a) North Side of Pier - Employees were unloading steel railroad rails from the hold of the M/V Pacific Light and placing the load onto a flatbed, where the load shifted and fell from the flatbed and landed on an employee, causing serious injury, on or about 03/18/2018.     No abatement certification or documentation required for this item.
