29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) East Main Street, Fleetwood PA - The employer failed to determine the exact location of an underground gas pipe by safe and acceptable means when the excavation operations approached the estimated location of the underground pipe, thereby exposing employees to struck-by injuries, on or about March 26, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
