29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:  On or about May 29, 2018, at jobsite located 18 Tahiti Island Beach Road, Coral Gables, FL 33143, an employee while mixing and installing pavers/tiles was exposed to Titan Ready-Mix Concrete (Portland Cement) a corrosive, without the use of hand protection.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about March 29, 2018, at jobsite located 18 Tahiti Island Beach Road, Coral Gables, FL 33143, the employer did not ensure that employees while working with a corrosive chemical, such as but not limited to, Titan Ready-Mix Concrete (Portland Cement), wore eye or face protection to prevent eye injury from splatter and flying projectiles when performing mixing and installing pavers/tiles.
