29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:  a) Jobsite - On or about March 27, 2018, employees were exposed to fall hazards of up to about 11 feet, 2 inches while using an aluminum extension ladder to gain access to the ceiling joists and top plates of a two-story home in that:  i) The side rails of the ladder were not extended at least 3 feet above the upper landing surface; and  ii) The ladder was not secured at its top to a rigid support that would not deflect and a grasping device was not provided for mounting and dismounting the ladder.
