On or about March 27, 2018, oxygen cylinders and fuel-gas cylinders in storage were not separated by a minimum distance of 20 feet or by a noncombustible barrier with a fire-resistance rating of at least one-half hour. Employees were exposed to the hazard of fire and/or explosion.
