29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Location: Garage A North; Garage A South, Garage B and Garage C Trash Compactors,    On or about 3/21/2018, the employer failed to protect each employee by developing, documenting and utilizing procedures for the control of potential hazardous energy. Employees performing maintenance (cleaning, unjamming) on trash compactors were exposed to caught in between hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about; 3/26/18    Location: Garage B,     a) Employees were exposed to struck by hazards due to a trash compactor having a malfunctioning safety interlock allowing the compactor machine to be operated with the door partially open (~3 inches wide opening), while not providing effective employee protection against the hazard.
