29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  (A)  On or about March 29, 2018, and at times prior thereto, a Metaltech 6' Baker scaffold was not inspected for visible defects by a competent person as required.  The scaffold had defects such as but not limited to, three (3) of the insert casters were tied with wire and did not have an effective means of locking them in place to prevent inadvertent movement of the scaffold.   Note: Abatement certification and documentation ARE required for this item.
