29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency was regained: a) Job site, 7215 McKnight Road, Pittsburgh, PA: On or about March 26, 2018, during construction activities, the employer did not ensure that proper scaffold retraining was performed where scissor lift safety concerns existed. Deficiencies noted included, but were not limited to, climbing on unit's guardrails/not ensuring that a safe means of scaffold access/egress was in use or not closing entry gates on the ends of the units, both of which occurred where fall hazards of to approximately 17 feet existed, and not ensuring that lifts were kept in good repair or were operated safely.
