29 CFR 1926.503(c): When the employer had reason to believe that any affected employee who had already been trained did not have the understanding and skill required by paragraph (a) of this section, the employer did not each such employee: a) Job site, 7125 McKnight Road, Pittsburgh, PA: On or about April 6, 2018, during roofing activities, the employer did not ensure that employees were provided with fall protection retraining where fall hazards of up to approximately 20 feet existed. Deficiencies noted included, but were not limited to, working outside the flag warning line system without appropriate fall protection systems/devices in use in that at least 2 employees worked outside the flag warning line system with no fall protection in use and another employee wore a harness and was tied off, but did not adjust the rope length for the rope grab while walking along the edge of the roof.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record that contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer or if the employer relied on training conducted by another employer or completed prior to the effective date of this section, the certification record did not indicate the date the employer determined the prior training was adequate rather than the date of actual training: a) Job site, 7125 McKnight Road, Pittsburgh, PA: On or about March 26, 2018, during roofing activities, the employer did not ensure that appropriate fall protection training records existed for all employees on the roof.
