Deficiencies noted included, but were not limited to, working outside the flag warning line system without appropriate fall protection systems/devices in use in that at least 2 employees worked outside the flag warning line system with no fall protection in use and another employee wore a harness and was tied off, but did not adjust the rope length for the rope grab while walking along the edge of the roof.
