29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about March 30, 2018, the crew leader and employees used the portable aluminum extension ladder that did not extend 3 feet above the roof landing surface when they accessed the roof to attach the roofing materials, being exposed to a 10-foot 5-inch fall hazard.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a) Employees performing roofing work on the 5:12 pitch roof, using pneumatic nailers are not being protected from being struck by nails and flying debris. The employer fails to assure effective protection against flying particles as required by the standard, as demonstrated on March 30, 2018, when 3-employees installing asphalt roofing shingles on the roof of the residence located at 6702 Azalea Park Road, Jacksonville, FL 32221, were not protected from flying particle injuries by the use of eye protection.    Coastal Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1274593, citation number 01, item number 001 and was affirmed as a final order on January 31, 2018, with respect to a workplace located at 9939 Patriot Court, Jacksonville FL 32221.    Coastal Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 951792, citation number 01, item number 001 and was affirmed as a final order on March 18, 2014, with respect to a workplace located at 4696 Karsten Creek Dr., Lot #326, Jacksonville FL 32208.    Coastal Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 948481, citation number 01, item number 001 and was affirmed as a final order on March 18, 2014, with respect to a workplace located at 4728 Plantation Oaks Blvd., lot 155, Jacksonville FL 32212.

Employees performing roofing work on a roof six feet or more above the ground are not protected from fall hazards by a fall protection system.     The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 30, 2018, when employees were installing shingles on the 5:12 pitch roof of a two-story residence at 6702 Azalea Park Road, Jacksonville, Florida 32259 without the use of complete fall protection systems and were exposed to fall hazards of 9 feet.    Coastal Roofing, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1274593, citation number 01, item number 002 and was affirmed as a final order on January 31, 2018, with respect to a workplace located at 9939 Patriot Court, Jacksonville, Florida 32221.
