29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    WORKSHOP:  The employer had not established and implemented written step-by-step energy control procedures in the machine specific including alternative measures for the employees during servicing, maintenance, and paper set-up changes on the equipment, such as (but not limited to) Goss Media 1-FR18301 printing press.      On/or about March 23, 2018, an employee's left elbow was pulled in between the drive/drag and the slitter during set-up change over the web lead paper on the above equipment, and the employee sustained injuries to the left elbow from the incident.
