The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On March 22, 2018 an employee while removing glue that had built up on Line # 4 in the knife cutting area of the line were not required to: (1) Turn off and shut down the moving parts on line #4 (2) Physically locate and operate energy isolating devices in such a manner as to isolate machine or equipment from the electric energy sources. (3) Affix lockout or tagout devices to energy device(s).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) Line #4 did not utilize a guarding methods such as but not limited too, barrier guards or electronic safety devices that would prevent employees from entering into the moving parts of the machine including but not limited to, the knife cutting area when the build up is being removed, during the hand spraying of alcohol and oil, exposing employees to moving parts.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. a) Horizontal belt on Line #4 attached to the gearing was not guarded, exposing employees to pinch point hazards.
