29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: a) At the site, where employees performing roofing work approximately 25 feet above a lower level were not protected form falling by any type of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the site, on the south wall, a Werner 28-foot extension ladder (Model D1328-2) was used to access the work area without extending at least 3 feet above the walking/working surface, exposing employees to fall hazards.
