29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  At the west side of the building, the employer did not ensure that employees performing masonry operations from the tubular welded frame scaffold were protected from a fall hazard of approximately fifteen (15) feet in that there was no guardrail system or other type of fall protection in use.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would  automatically square:   On the west side of the building the employer did not ensure that the scaffold frames and panels were fully braced to secure vertical members together as required. Employees were exposed to a fall hazard of up to fifteen feet should the walkways become dislodged due to the lack of sufficient cross braces.
