At the west side of the building, the employer did not ensure that employees performing masonry operations from the tubular welded frame scaffold were protected from a fall hazard of approximately fifteen (15) feet in that there was no guardrail system or other type of fall protection in use.

Employees were exposed to a fall hazard of up to fifteen feet should the walkways become dislodged due to the lack of sufficient cross braces.
