29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide:   A)  South side of building, on or about March 29, 2018, employees were working from the first and second tier of a two tier scaffold that were not fully planked, exposing the employees to a 6 foot and a 12-foot fall hazard.   B)  North side of building, on or about March 29, 2018, employees were working from the second tier of a two tier high scaffold that was not fully planked, exposing the employees to a 14-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:   A) South side of building, on or about March 29, 2018, employees did not have a ladder or other safe means to access and exit the first and second tier of a two tier high scaffold, exposing the employees to a six-foot and twelve-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A) North side of building, on or about March 29, 2018, employees were using a metal extension ladder to access the second tier of a two tier scaffold. The ladder did not extend three feet above the scaffold platform, exposing employees to a 14-foot fall hazard.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   A) South side of building, on or about March 29, 2018, employees were working from a scaffold platform on the second tier of a two tier high scaffold that did not have guardrails on the open sides exposing the employees to a 12-foot fall hazard.   B) North side of building, on or about March 29, 2018, employees were working from a scaffold platform on the second tier of a two tier high scaffold that did not have guardrails on the open sides, exposing the employee to a 14-foot fall hazard.
