29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  (a): Seiko Complex- The employer did not ensure that each affected employee wore a protective helmet when performing tree trimming work in the presence of falling branches. An employee was injured when he was struck on the head by a falling branch. The employer failed to provide a protective helmet to the employee before he was injured, on or about March 28, 2018.  There is no abatement certification or documentation required for this item.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours.  a) Seiko Complex- The employer did not report the in-patient hospitalization of an employee to OSHA within twenty-four (24) hours. OSHA was not notified by the employer of an accident that resulted in the hospitalization of an employee that occurred on March 28, 2018. The accident was reported to OSHA by local police.
