29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about April 12, 2018, in the back of the home under new construction, employees were working on a deck without fall protection, exposing the employees to a fall hazard of approximately 10  feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    Instance 1- On or about 30 March 2018, at the above addressed jobsite, an employee working from an "A" Frame step ladder was standing on the top hat of the ladder, exposing the employee to a fall hazard of approximately 9 feet.    Instance 2- On or about April 12, 2018, CSHO returned to the above addressed jobsite and observed another employee work from an ''A'' Frame step ladder while standing on the top hat of the ladder.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about 30 March 2018, at the above addressed jobsite, an employee working from an "A" Frame step ladder was standing on the top step of the ladder without the employer providing ladder safety training, exposing the employee to a fall hazard of approximately 9 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  On or about April 12, 2018, in the back of the home under new construction, employees were accessing the tubular welded scaffolding by climbing the outside rails and not utilizing a ladder, exposing the employees to a fall hazard of approximately 10  feet.
